identification cards policy and procedures

id cards are to be visible at all times.

100% id check.

requirements: students must be registered for classes and possess a valid state or government issued id (passport, drivers license, state id card, immigration card), in order to obtain a york college identification card.

new student

1. complete a york college id application form.
2. verify cuny first information.
3. take photograph.
4. issue card to student.

staff / faculty

1. id application form must be accompanied by hr id request form .
2. take photograph.

• faculty electronic access id cards will be programmed  

lost id card replacement procedure

student/staff

1. department will check for returned cards.
2. student info verified on cuny first.
3. lost id replacement form completed with approved public safety personnel initials.
4. $10.00 replacement fee paid at bursar.
5. * upon receipt of proof of payment, a replacement id card will be issued

faculty/staff with access cards

1. lost id cards must be reported immediately to the public safety department, access will then be revoked.
2. id card will be issued after lost id card replacement procedure *(1e) is completed.
3. lost card replacement form completed with approved public safety personnel initial.  
4. $10.00 replacement fee paid at bursar.
5. replacement card will be issued.
